Appendix A- Reminders to CHEER Members & Participants
1.All ethnic minorities can enroll in our services by simply calling the TELIS Hotlines or visit our Centre during operation hours for interpretation and translation services, counseling guidance and referral services.
1.No fee is required for the application of Service User Cards.
2.Renew of Service User Card is required when the card is lost or damaged.
3.Service Users could exit our services by giving verbal request for termination to our staff.
2.Only CHEER’s registered users can join CHEER’s programmes by completing programme application forms.
3.You should take care of your personal belongings and property.
4.Parents and guardians are responsible for their own safety and the safety of their children. Parents and guardians should keep an eye on their young children to protect them from possible harm.
5.Report immediately and seek help from CHEER staff if you encountered any accident or injury in any CHEER activities.
6.All CHEER users must comply with all rules, regulations and laws of CHEER and Hong Kong. Illegal behavior and action will not be tolerated.
7.Personal Data Collection Statement
1.The personal data you have provided will be used by us for the provision of those services and programs which you have enrolled, and promotion of other services and programs offered by us from time to time. The personal data may also be disclosed to our sub-contractors, agents, professional advisers, the HKSAR Government and other service providers from time to time to be appointed by the HKSAR Government in our place for the provision of services to you. In addition, the HKSAR Government may also use the personal data for the purposes of conducting research and surveys, monitoring the services which we provide, and enforcing the project agreement between us and the HKSAR Government in relation to the services, and all other purposes arising from or incidental to the aforementioned purposes. The personal data provided may be disclosed by the HKSAR Government to other public bodies or such other person as the HKSAR Government considers appropriate for any of the aforesaid purposes.
2.By submitting this application, you are regarded to have agreed to the disclosure and use of personal data as specified in the Paragraph 7.1 above.
3.You have the right of access and correction with respect to your personal data as provided for in sections 18 and 22 and Principle 6 of Schedule 1 to the Personal Data (Privacy) Ordinance (Chapter 486 of the Laws of Hong Kong). The right of access includes the right to obtain a copy of your personal data provided in the application.
4.Enquiries concerning the personal data collected by means of this application, including the making of access and corrections, should be addressed to our Centre-in-charge.
